Filipino citizens are entitled to a Philippine passport. Filipino citizens who have acquired Canadian citizenship are eligible to apply for a Philippine passport after re-acquisition of their Philippine citizenship (oath of allegiance under RA 9225).  A passport may be renewed 6 months prior to its expiration date. 

Personal Appearance  & a 
confirmed appointment schedule are REQUIRED for APPLICATION OF A NEW PASSPORT (PLEASE COME IN PROPER BUSINESS ATTIRE – WITH COLLAR AND SLEEVES).

 

I. PASSPORT RENEWAL

  • A. Latest passport with photocopy of the data page (page showing photo & complete name)

  • B. Duly accomplished passport application form

  • C. Original and photocopy of one of the following: Applicant’s valid Permanent Resident Card/Work Permit, Dual Citizenship Certificate (Order of Approval/Identification Certificate), Status Verification from CIC, Record of Landing (if within 3 years from date of arrival),  or latest Tourist Visa to Canada, whichever is applicable

  • D. If the latest passport currently held is a Green Passport or a Machine Readable Passport (MRP) or was issued on or before 01 May 1995, submit the following:

    • PSA / NSO issued Birth Certificate (original & photocopy);

    • At least one (1) valid identification document (ID);

    • Other supporting documents (i.e. PSA/NSO marriage certificate if married and using married surname, etc.); and

    • Fee: C$81.00 for new passport.

   

Change of Name (for female applicants) from Maiden (Single) to Married

​          Requirements A, B, C, D plus

  • E. If married in the Philippines - Original & photocopy of NSO Marriage Certificate and Birth Certificate;

  • ​ F. If married in B.C., Alberta, Yukon, NWT - Original & photocopy of NSO Marriage Certificate and Birth Certificate.  Fees:C$81.00 for new passport; C$33.75 for Report of marriage.

  • ​ G.If married outside of B.C., Alberta, Yukon & NWT - Original & photocopy of NSO Marriage Certificate and Birth Certificate.  ​Fee: C$81.00 for new passport.

       Note: For those who were married outside the Philippines, a Report of Marriage (ROM) must have been filed with the Philippine Embassy/Consulate with proper jurisdiction for the Philippine Statistics Authority (former NSO) to be able to issue an authenticated Marriage Certificate.

 

​Change of Name (for female applicants) from Married to Maiden (Single)

​        Due to Death of Spouse

        Requirements A, B, C, D, E plus

  • H. Death certificate from NSO of the deceased husband; or Court Order for presumptive death.  Fee: C$81.00 for new passport

        Due to Annulment/Divorce: (those recognized under Philippine Laws)

        Requirements A, B, C, D, E plus

  • I. Duly authenticated copy of annulment/divorce decree/order and NSO-issued MC with the annotation on the annulment decree; *Divorce should be duly recognized by a Philippine Court.  Fee: C$81.00 for new passport

      Married applicants who opt not to use husband's surname (allowed if husband's surname has not been used before).

       Requirements A, B, C, E, if latest passport is green ; A,B,C,D,E or F, if latest passport is brown, plus

  • J. Affidavit that applicant has opted to use maiden surname.  Fee: C$81.00 for new passport C$33.75 for Report of Marriage (if no report has yet been made).

​II. REPLACEMENT OF LOST PASSPORT

(Please note that there is a 15-day verification prior to the processing of application for replacement of lost valid passport)

     Requirements B, C, D, plus

  • K. Police Report (if lost passport is still valid)

  • L. Affidavit of Loss ( may be accomplished at Philippine Consulate General for separate fee)

  • M. Duly authenticated birth certificate

  • N. Duly authenticated marriage certificate (for married women applicant)

  • O. Photocopy of latest/lost passport, if available

  • Additional Requirements: E or F, H, I, J (whichever is applicable)

  • Fees:    C$202.50 for lost e-passport/C$121.50 for lost MRP or green/brown passport.

III. PASSPORT APPLICATION FOR MINORS (under 18 years old)​

​     First Time Applicants

        Requirements B, C plus

  • P. Birth certificate issued by the Vital Statistics of Canada

  • Q. Marriage certificate of parents and proper ID (if minor is legitimate status); Personal appearance of Mother and mother’s ID (if minor is of illegitimate status)

  • R. Passports or any government issued IDs of applicant’s parents and photocopies of such ID

  • S. Letter of consent signed by either parent in the presence of the Consular Officer.  If neither parent can appear in person, the signature/signatures must be acknowledged by a notary public

  • T.  Report of Birth

  • Fees:        C$81.00 for new passport; plus C$33.75 for report of birth

​PASSPORT RELEASE

Applicant MUST provide a self-addressed prepaid return envelope per applicant from CanadaPost (secure a signature on delivery sticker  if possible) or a private courier service of applicant’s choice.  The Consulate however will NOT assume responsibility for passports/documents that may be lost in the mail.

 

​MODE OF PAYMENT

Vancouver applicants may pay in exact amount in cash or by money order/bank draft payable to the Philippine Consulate General.

Applicants in an Outreach Program MUST pay by MONEY ORDER or BANK DRAFT only, made payable to the Philippine Consulate General.
