REQUIREMENTS FOR THE REPORT OF BIRTH

 

The birth of a child born in British Columbia, the Yukon and the Northwest Territories of Filipino parents should be reported to and registered with the Philippine Statistics Authority through the Consulate General.


When reporting/registering the birth of the child, the original documents should be submitted along with four photocopies. The following are the requirements:

FOR CHILD WHOSE PARENTS WERE MARRIED AT THE TIME OF CHILD’S BIRTH FOR CHILD WHOSE PARENTS WERE NOT MARRIED AT THE TIME OF CHILD’S BIRTH FOR CHILD WHOSE PARENTS WERE MARRIED ONLY AFTER THE CHILD’S BIRTH

1. Report of Birth form (4 originals).

2. Birth Certificate with parental information/Registration of Live Birth from the Vital Statistics Office.

 

3. Authenticated Marriage Certificate of Parents from the Philippine Statistics Authority (PSA), if applicable.

 

3. IF THE CHILD IS USING THE SURNAME OF THE FATHER:

Pursuant to the Revised IRR of R.A. 9255, 3.a. the father shall execute an Affidavit of Admission of Paternity (AAP)

3.b. the mother/guardian shall execute an Affidavit to Use the Surname of the Father1 (AUSF1)

3.c. the mother/guardian shall execute a Sworn Attestation, if applicable

Note: If the two (2) affidavits will be executed in the Philippines, these should be registered first as Legal Instruments at the Local Civil Registry Office of the place of execution. If executed abroad, these should be registered first at the Foreign Service Post (FSP) of the country of residence of the parent(s) or at the FSP nearest the place of residence.

IF THE CHILD IS USING THE SURNAME OF THE MOTHER:

3.d. The mother shall execute an Affidavit of Illegitimacy and also submit her PSA-issued Birth Certificate

Note: There shall be no entry for the item on the child’s middle name.

3. Affidavit of Legitimation by Subsequent Marriage(with PSA-issued Birth Certificate of mother)

4. Passport of Parents (valid at the time of the birth of the child).

    - Bio data page only. 

5. Parent/s’ proof of status i.e. permanent resident card, copy of visa, working permit if working abroad at the time of birth of the child or identification certificate (if dual citizen). 

6. Four (4) recent passport size (4.5cm x 3.5cm; colored with white background) photos of the child/subject.

 

7. Processing fee for Report of Birth: 33.75 CAD in cash or money order/bank draft made payable to the Philippine Consulate General.  Do not send cash in the mail; PLEASE NOTE THAT EACH AFFIDAVIT OR CERTIFICATION HAS AN ADDITIONAL FEE OF 33.75 CAD. 

8. Self-addressed envelope.

 

ADDITIONAL REQUIREMENTS FOR DELAYED REGISTRATION

9. First passport or Travel Document of the Child/Subject.

    - Photocopy of bio data page. 

10. Notarized Affidavit of Delayed Registration of Birth.

If the parent/s intend to have their child issued a Philippine passport, they need to submit the following documents in addition to the requirements for the ROB application:

  1. Duly accomplished passport application form
  2. Report of Birth
  3. Certificate of Birth issued by the Vital Statistics Office
  4. Marriage Certificate of Parents, if applicable
  5. Passport bio data page or any government issued IDs of parent/s of the child
  6. Parent/s’ proof of status i.e. work permit, PR card, visa, identification certificate (if dual) etc.
  7. Self-addressed envelope

In this case, the child will also be required to be physically present at the consulate general when applying for the passport.

The Consular Officer reserves the right to require additional proof or documents from an applicant to prove his/her citizenship, status or identity pursuant to the Philippine Passport Law (RA 8239) and the Foreign Service Act (RA 7157).

ALL APPLICATIONS SUBMITTED BY MAIL OR BY A REPRESENTATIVE SHOULD BE DULY NOTARIZED.


IMPORTANT REMINDER: Applicants must comply with the above requirements. INCOMPLETE REQUIREMENTS will not be processed and returned to the applicant.

 1 If child is 0-6 y/o: AUSF to be executed/signed by mother

   If child is 7-17 y/o: AUSF to be executed/signed by the child with sworn attestation by mother/guardian

   If child is 18 y/o and above: AUSF to be executed by child with no required sworn attestation
